On-Line Entry Form

This form replaces all paper forms for your entries except for bottle labels.
Please read and understand these instructions completely before proceeding. Refer to the detailed Rules for Entering if necessary.

You will then be given a unique ID number for each entry to attach to your bottles. It is imperative that this number be attached to your bottles before submitting them to the contest in order to ensure that you entry is judged in the correct category. The confirmation page generated automatically when the form is submitted includes bottle labels which can be printed, cut out, and attached to your bottles.
If you have more than one entry to register, use the BACK button of your browser to return to this page for each subsequent entry and update the necessary fields on the form.
